I have made a few forms that could be useful to many of you. Please feel free to use them. All forms are in PDF format.

Emergency Recovery Sheet

KeePass. Use this form for KeePass to record your password, email address and email password and other essential information my might forget or be unable to access. Instructions are here.
Bitwarden. This can be used for any online password manager. This is for storing critical information in printed form. It is a PDF fillable form.
